Just in Time Care and Care Coordination
Often, due to the normal aging processes, older adults need time, accommodations, and thoughtful explanations of how to self-care manage medications, wounds, oxygen equipment, glucometers, self-injections, accessing health supplies, and problem solving some of the social determinants of health that impede self-care management within their own daily story.​​​​

In this volunteer role, Wisdom Wellness clinicians provide those insights each week. Within the scope of licensure and based on the seniors plan of care, our licensed clinicians provide expertise to support the seniors self-care management in their home. Our no-fee services are not constricted by number of visits rather by what the person needs; hence giving time and space for whole-person centered relationship based care. At times, there is triaging, advocacy, and facilitation work that the clinician does which best positions the senior to get access to their health-benefit coverage, primary care physician, specialists, etc. And sometimes, we mitigate a care gap that if left unattended would negatively affect the senior’s health and ability to self-care manager.
Time: 2-4 hours per week, every other week or monthly. Current need is Tuesday afternoon.
Talent: Volunteers* are NP, RN, LPN, CNA, HHA, CHW, Nursing Assistance, Social Workers, Physical Therapist. Helpful skills are: you enjoy working with seniors, have a patient listening heart, are willing to help problem solve with the senior, experienced with taking vital signs, understand basic first aid, and are comfortable with light charting of the senior’s visit.
*Volunteers can be actively employed, retired and or students.
Frail Elder Visitor
​Our "Frail Elder Visitor" program is a set of services and supports designed to help the elders who are physically frail to continue living independently in their own apartment.​​ it gives the opportunity for the elder to have conversations with clinical and non-clinical volunteers, review their weekly care plan, medical monitoring, care coordination, medication reviews, social/emotional support, problem solve any emerging health questions, and promote the elder’s autonomy and quality of life in their self-care management.
Time: 2-4 hours, per week, every other week or monthly. Current need is Tuesday afternoon.
Talent: Volunteers can be clinical and non-clinical, all are welcome (e.g., NP, RN, LPN, CNA, HHA, CHW, Anyone interested in helping). Helpful skills are: you enjoy working with seniors, have a patient listening heart, are willing to help problem solve with the senior, experienced with taking vital signs, and are comfortable with light charting of the senior’s visit. For non-clinical volunteers we can teach vital sign capture and light charting.
Social and Functional Caring
Listening hearts provided by our volunteers plays a vital role in the well-being of all who are served by Wisdom Wellness, especially those seniors who may be isolated or living alone. Friendly conversations, shared activities, and the opportunity to revisit the same topics with weekly connections provided by Wisdom Wellness can brighten a senior's day and improve their overall mood. Regular social interaction can enhance a senior's quality of life by providing mental stimulation, reducing stress, and promoting a sense of belonging within the community. This can lead to improved cognitive function and better emotional health. Getting to know the elder’s personal stories, providing 1:1 personalized attention that is not limited to a time constrained office visit fosters trust and mutual respect, creating a supportive environment where seniors feel valued and understood. Ultimately, these interactions are the cornerstone of healing and wellbeing every elder benefits from.

Pharmacy Medication Pick Up
By offering your time to ensure that someone receives their medication supplies promptly and safely, you are providing crucial support and demonstrating compassion in action. Your willingness to volunteer for such a task not only helps the individual in need but also contributes to their overall well-being and peace of mind.These medications are not controlled substances.
Time: 1-1.5 hours one time per month or every other month; current need is for a Tuesday afternoon between noon-3:00 pm
Talent: A volunteer is needed to pick up monthly prescription orders from one central pharmacy and drive it to the Wisdom Wellness site for our clinical volunteers to distribute. This is a non-senior contact role. Your contact is with a pharmacist and Wisdom Wellness nurse. You must be 18 plus.​
Interdisciplinary Professionals
Wisdom Wellness creates a valuable synergy between different generations and professional backgrounds to support the older adult in managing their self-care effectively. Hidden challenges like isolation and loneliness that go unseen between traditional clinical visits are mitigated with weekly team contact by our amazing volunteers. By combining the clinical wisdom of current and retired professionals, with the new perspectives and energy of emerging professionals and students, we offer a rich environment for shared community support.
