Providing Just in Time Care
Delivering just in time care for chronic illness self-care management which is reviewing/reminding and reengaging the senior in their self-care management tasks. Mitigating social, functional, and physical challenges lived everyday by the population addressing real time issues as they arise, and often call for long-term repetitive supports.

Longitudinal commitment
Staying connected to what the elder needs to accomplish positive self-care management overtime; Wisdom Wellness is guided by the elder's time, needs, interests, culture, treatment plan, and decision-making.

Helping to remove barriers to everyday living.
Identifying the senior’s individual strengths and barriers to living well independently each day in their home, and staying present to the small incremental changes that can improve or diminish health and safety. Celebrating strengths and accomplishments in self-care management.

Resource identification and action plan creation.
Identifying existing resources, (financial, human, supplies, insurance, PCP, specialist care, etc.), that are available to address any, some or all the items identified in #1-3.
Creating individualized action plans for the elder that a Wisdom Wellness volunteer walks through weekly with the senior putting into real time solution action from learnings.

Mentoring
Creating learning and mentoring opportunities for emerging health care leaders- nurses, physical therapist, nutritionists, community health workers, students, and all interested in community health care.
