We Offer

Volunteer nurse practitioners, registered nurses, and community health workers provide their combined expertise and time on-site, one afternoon a week, at low-income independent senior living communities, addressing elders existing and emerging health challenges; our services* are free to the residential low-income senior.

Helping Hands

Whole Person Care

Social, Emotional, Functional

  • Listening, validating, advocating, facilitating, triaging, and referral care coordination

  • Explanation of treatment plans

  • ​Glucometer Trainings

  • Help with understanding a prescribed health product (e.g., inhalers, oxygen,, braces, in-home health monitoring equipment, bowel preps, etc.

  • Pharmacy delivery program

  • Safety Checks within the residential community

  • Covid and flu education

  • General health education-"Talk Circles'; Lunch 'n Learns

Physical Care

  • Vital signs | weight check

  • Wound care assistance

  • Diabetic foot check

  • Monitoring injection therapies

  • Medication reconciliation

  • Medication safety set up

  • Range of motion

  • Physical Therapy education supports and coordination

  • Ambulation and mobility strengthening

  • Health assessment and preventative screenings

  • Monitoring over the counter self-care management testing (e.g., Covid, UTI, blood sugars, etc.)

  • Healing Touch

  • *All services and assistance are within the volunteers scope of practice; all follow the elder's individual treatment plan by their care provider, under the guidance of best practice protocols, and an onsite clinical practice lead.
