We look forward to hearing from you and having you join our mission to support seniors to live independently in their home leveraging community resources, existing health insurance, and supportive partnerships.

Q. How much do the Wisdom Wellness Services cost the senior and do you accept insurance?
Our services are provided at no cost to the senior and there is no insurance necessary for our services. As a matter of fact, our volunteers spend a lot of time assisting the senior with accessing their full benefits, connecting the senior to their primary care physician, and community support services as they self-care manage and live independently.

Q. I understand Wisdom Wellness Services are free? How does that work?

Since beginning in 2017, our care model has relied on the generosity of volunteers that are a dedicated group of individuals with a diverse range of clinical skills (licensed), community understanding (CHWs), diverse professional backgrounds, and a deep collective commitment to social caring. All volunteers are fueled by a desire to make a tangible difference in the lives of those in need, utilizing both their field of expertise and their empathetic nature. Additionally, to date we have been enriched by community partners and angel donors who have given supplies and funds to support Wisdom Wellness in the community care work.

It’s very simple, time and continued engagement weekly (years). A care coordination model that is continuous in real time for the senior.

Wisdom Wellness care begins with listening to understand what matters most to the senior based on the senior’s lived experience; it is not the number of visits, diagnosis or time driven, rather the senior decides when, how often, and for what reasons they come to Wisdom Wellness.

We believe what makes the difference is that our volunteers have a belief in the power of human connection, and a practice centered in the importance of holistic relationship based care that is longitudinal. This type of time commitment, visiting with the same senior on the same social, functional, and physical needs, concerns, and challenges, week in and week out, is not part of the usual practice settings or benefit plans our seniors have access to.

Q. How long are visits when a senior comes to the Wisdom Wellness service space?

Our volunteers provide the time and attention to the senior's requests to have their health questions addressed. Being able to visit Wisdom Wellness weekly, to ask the same or similar questions about their health, treatment plans or general questions on how to self-care manage their complex chronic conditions, is what sets Wisdom Wellness apart from traditional health care coordination programs or home care services. Wisdom Wellness is available weekly and not connected to or driven by "a" diagnosis or "defined by allocated allowable visits", usually based on a diagnosis or an evaluation that determines the senior no longer qualifies for assistance. The elder's voice and vision for their self-care management in their time and place is what drives Wisdom Wellness work.

Q. What is the Wisdom Wellness Care Model and how are you different from the care a senior already has?

Some volunteers come through community partnership and some come from local Universities. We are always looking for volunteers and with more volunteers we would be able to expand days of the week at our current sites and possibly add additional sites.

Q. Do you accept donations?

YES! THANK YOU for asking! Wisdom Wellness is a charitable non-profit with the State of MN with 501 (c) (3) (pending) formed to support the older adult to live safely in their home. Soon we will have a direct on-line donation link; in the meantime, you can send monetary donation payable to Wisdom Wellness, Inc. directly to First Resource Bank | 811 South Washington Avenue | Suite 104 | Minneapolis, MN. 55415.

Q. Would you consider establishing the Wisdom Wellness Model at other sites or helping my team get started?

Absolutely! Over the seven years of growing the learning, research, and evaluation of the model, we have established templates, training, procedures, volunteer structure, etc. for our work. Technical consultation and customization work to roll out a Wisdom Wellness program in your community or organization is possible.
